These forms are provided to assist with various pursuits.   From submitting an application for employment with the police department, to requesting that a local road be closed for a party, to inquiring about your rights as a victim, and more . . . these forms are for your convenience.   Some forms use Adobe Acrobat Reader and other are in Omni Form Pro and can be filled out on your computer after downloading same.   Please advise of any problems you find by sending an email to the Chief at:

Employment Application Sworn Officer

Employment application used to apply with the police department in both sworn and non-sworn positions.   Lake Placid Police Department is an Equal Opportunity Employer.  This packet will download to your computer when clicked.   Once on your computer it can be filled in, saved, printed, and easily and neatly presented for employment.   This format greatly expedites your background investigation since the application is easily read by the background investigator.
 

Emergency Contact Form  

If you are a new business or someone wanting to let us know how to contact you in case of emergency (for example if you have and elderly parent that lives in the Town of Lake Placid) use this form. 
 

Citizen Request  

Use to request house or business watch for a period of time, use to request a report or make a public records request, use to file comment of a police officer.   An important program to the Lake Placid Police Department is the K.I.S.S. program (Keeping Inbound Seniors Safe).  Use this form to register elderly persons living in the town with the police department.  Last Winter the department immediately checked on the 8 registered K.I.S.S. seniors when the temperature was forecasted to drop below 25 degrees.
 

Victims Brochures  

If you are a victim of crime in Florida this brochure, approved by the Governor of Florida's Victim Advocate Office, advises of your rights.
 

Victims Notification  

If you are a victim of certain crimes you may exercise your right not to have your name released pursuant to Florida's public records law.   Using this form you may log your request with the police department.

Worthlass Check Packet

 

A favorite form for high volume businesses, this form is required in the 10th Judicial Circuit of Florida (Lake Placid included) to file worthless check charges.  This form has been upgraded to Adobe pdf format and has fillable form fields.   Once on your computer it can be filled in and printed neatly.   This format greatly expedites case prosecution.
 

Road Closure Request  

Used to request road closure.  More importantly, the information in this form is used so police can plan what equipment is needed and how much notification the public needs if any. 

 

Traffic Accident Request  

New Florida laws requires the department to acquire specific identity and intentions for use of the report from persons wanting copies of traffic accident reports.   Whether you request an accident report in person or by mail the department will require an ID and completion of this form.

Alcohol Ordinance Variation Request  

Request a variance in alcohol ordinance for consumption outside

Formal Statement  

This form is used for persons to submit formal statements to the police where person is a victim, a witness, or where a citizen desires to submit a report regarding any of the police department's actions.   

Suspicious Activity  

One of the trademarks of an illegal drug trade is suspicious frequent short term visitors to a residence.   When notified the police investigate such activities thoroughly.   Information furnished on this form can help us determine the character of the individuals that are visiting.  Example: If license plates submitted on your form is registered to an individual who has a criminal record of past drug experiences our ability to act to stop the problem is enhanced.

Witness Statement

 

Use to furnish a formal statement to police, to report additional facts or property stolen.  Use to report police misconduct, to report a code violation, or any other details.

 

RECREATION VOLUNTEER CRIMINAL HISTORY FORM

 

If you are volunteering to be a coach or to be involved in Lake Placid Recreation for the Town of Lake Placid, fill out this form.  Town Recreation Director John Komasa with the Town Recreation department needs a completed copy.  Bring another copy with you to the Highlands County Sheriff's Office located on Fern Avenue in Sebring.    Additional instructions are on the form in the red square. 

When the form asks if you have been fingerprinted before it is referring to past qualified entities.  Real Estate, Nursing, Military, Past Jobs, are not qualified entities.   Qualified entities are entities registered with VECHS.   

This service is provided for your community by the Highlands County Sheriff's Office and Lake Placid Police Department to help keep your children safe.     

 

CLOWN VOLUNTEER CRIMINAL HISTORY FORM

 

f you are volunteering to be a Clown with Toby's Clown College fill out this form.   Bring a copy of the completed for to Chief Williams at the police department (8 North Oak Avenue, Lake Placid, Fl. 33852) and bring a copy with you to the Highlands County Sheriff's Office located on Fern Avenue in Sebring along with a $5.00 bill.      Additional instructions are on the form in the red square. 

When the form asks if you have been fingerprinted before it is referring to past qualified entities.  Real Estate, Nursing, Military, Past Jobs, are not qualified entities.   Qualified entities are entities registered with VECHS. 

The Sheriff's Office will bill the clown college $33.25 for each person fingerprinted.   Payment to the clown college is your responsibility. This service is provided for your community by the Highlands County Sheriff's Office and Lake Placid Police Department to help keep your children safe.   

 

HIRE AN OFFICER

The Police Department provides policing for many public events.   For private events, depending on the character of the event, policing is available for hire.   The event must be inside the town's limits.   The Chief of Police retains authority over all police activities and police must follow Lake Placid Police Guidelines when working at special events.   After completion relay the completed for to the Chief of Police by mail, personal delivery or email.
